- The central slip of the extensor tendon attaches to the base of the middle phalanx, playing a crucial role in extending the PIP joint of the finger.

- The central slip can be lacerated, ruptured or attenuated secondary to disease like rheumatoid arthritis.
- Injury to the central slip can cause a boutonniere deformity by decreasing PIP extension and by allowing the lateral bands to be displaced volarly and below the axis of PIP motion.
